Advance Care Agency is a certified NDIS Provider. Our team of registered nurses and skilled healthcare professionals provide in-home nursing to help manage and coordinate recovery at home. We deliver professional health service solutions to facilities quality home care whilst training existing health professionals and educating a new generation of health support staff. Essential Requirements:
- Current NSW Driver’s License (No International or Provisional Licenses).
- CPR and First Aid Certificate.
- Minimum Certificate III in Individual Support or Equivalent
- Current Police Check clearance.
- NDIS Worker Orientation Module Certificate and NDIS Worker Screening number.
- Full working rights within Australia.
- Effective oral and written communication in English.
- Ability to write comprehensive reports and perform administrative tasks.
- Qualification in Aged Care/Disability Course.
- Experience preferred in the disability field and/or aged care.
Main Duties/Responsibilities:
- Support clients in age-appropriate activities, employment, in-home support, and community access.
- Provide direct assistance according to clients’ service plans.
- Adhere to medication administration policy, work health and safety procedures, and client rights policies.
- Monitor and report on the safety and well-being of clients.
- Ensure proper documentation in client notes and communication books.
- Participate in Lifestyle Service Planning and monitor outcomes.
- Collaborate with clients’ families, carers, case managers, and other professionals to support individual goals.
- Develop, implement, and evaluate training and behavior programs.
- Maintain, document, and review all programming.
- Serve as a key worker for designated clients, including preparing monthly progress reports.
- Liaise with other staff to ensure smooth and consistent service delivery.
- Report regularly to the manager and participate in team meetings, planning, and policy reviews.
- Provide backup and support to other staff when necessary.
- Participate in Performance Development meetings for ongoing training and support opportunities.
- Notify the service of any inability to work rostered shifts with as much notice as possible.
- Contribute to case reviews with Employment Consultants and Facilitators.
- Take reasonable care for personal health and safety and ensure actions do not adversely affect others’ health and safety.
- Report any quality and safety incidents or hazards at work to the manager or supervisor.
- Carry out roles and responsibilities as detailed in relevant policies and processes.
- Stay fully informed about the NDIS system and apply it to daily work.
Additional Information:
- Flexible hours available.
- No experience required for this role.
- Work visa can be provided for this role.
- Expected salary: $75,000 – $78,500 per year.
- Hiring: 10 to 15 Full-time Disability Support Workers.
