Whether you’re leaving the hospital, switching providers or trialing independent living for the first time, our transition team handles:
1. Pre-move planning – NDIS paperwork, goal setting and housemate matching.
2. First-week intensive support – 24/7 nurse cover to manage medications, dressings and equipment set-up.
3. Ongoing reviews – monthly check-ins with you (and your circle) to adjust rosters or therapies as life evolves.
In the morning, wake up to the smell of your favourite breakfast. Practice meal prep skills with your Support Worker or let us handle it while you get ready.
Mid-morning – Hydrotherapy at Doone Kennedy Aquatic Centre or a coffee run on the waterfront. Afternoon – Art class at Salamanca Arts Centre, gaming with housemates, or chilling with a book.
